Connected Minds is a Nurse Practitioner–led clinic providing ADHD, neurodevelopmental, and mental health assessment and treatment for children (6+), adolescents, adults, and families.

We deliver neurodiversity-informed, evidence-based care within a framework of connection — recognising that mental health and development are shaped by relationships, family and whānau, culture, strengths, and lived experience.

Connected Minds exists to improve access to timely, high-quality care, helping individuals and whānau navigate assessment and support with clarity and confidence.

Based in Warkworth, we offer telehealth across Auckland and New Zealand, with in-person appointments available in Orewa and Warkworth. Appointments are available NOW. We currently DO NOT have a waitlist.

How It Works In 5 Simple Steps

Enquiry or Referral & Completion of Intake Documentation

An enquiry, self-referral, or referral from a GP or other professional is received. Prior to assessment, clients are provided with pre-assessment documentation. This step supports informed consent, clinical preparedness, and safe service delivery. There is a $60 non-refundable pre-assessment fee.

Review & Triage

The referral and completed forms are carefully reviewed to ensure the service is appropriate and that we have a good understanding of your situation. If Connected Minds is not the most appropriate service, clients are advised and supported with alternative recommendations where possible.

Appointment Booking

Once pre-assessment test payment and completed documentation are received and triaged, an appointment will be offered.

Assessment & Care Planning

The initial assessment is an important first step in the diagnostic process. It helps us understand the history, current concerns, and overall wellbeing, while also allowing time to build connection, rapport, and trust before moving into the diagnostic assessment. At the conclusion of the assessment, the clinician will discuss preliminary clinical impressions, any further assessment requirements and recommended next steps and management options.

Ongoing Support & Review

Depending on clinical need, further assessment may be required (ADHD/Autism/Diagnostic clarification. After diagnostic assessment/clarification, ongoing care may include: diagnostic feedback and formulation, written clinical reports (where indicated), medication initiation and monitoring and follow-up appointments and outcome review. Our NP can initiate and monitor medication treatment, including Special Authority applications where required, if this is indicated and consented. Care is reviewed regularly and adjusted based on clinical response and individual goals.

At Connected Minds, we recognise that neurodevelopment and mental health exist within relationships, family systems, and shared experiences. When a child begins their ADHD journey, it is common for parents to reflect on their own experiences — past and present. Gaining clarity can be empowering, validating, and deeply connecting for families.

As part of our commitment to holistic, whānau-centred care, we are pleased to offer ADHD assessments for parents of children and adolescents currently supported by Connected Minds. We are also able to provide ADHD assessments and medication support to those without children seen by Connected Minds.

Connected Minds is pleased to offer secure, professional online consultations for ADHD and mental health assessments and follow-up care.

Telehealth appointments allow individuals and families to access evidence-based, Nurse Practitioner–led care from the comfort of their own home — whether you are local to Auckland or elsewhere in New Zealand.

Online consultations are conducted via a secure platform and provide the same structured, clinically sound approach as in-person appointments.

Connected Minds is pleased to now offer in-person appointments in Warkworth and Orewa, alongside our telehealth services.

These face-to-face appointments offer an opportunity to connect in person and receive thoughtful, comprehensive care grounded in evidence and relationship-centred practice.

We currently do not have a waitlist, allowing families to access timely assessment and support when it is needed.

Jam Jones, MN, NP

Nurse Practitioner | Founder/Director, Connected Minds

Jam Jones is a specialist Child and Adolescent Nurse Practitioner working with children, young adults and their whanau from a private clinic based in Warkworth. She has extensive experience working across child, adolescent, adult, and perinatal mental health services in Aotearoa New Zealand.

Her clinical background includes work in:

  • Child and Adolescent Mental Health Services (CAMHS)
  • Adult acute and community mental health
  • Perinatal and maternal mental health
  • Neurodiversity and ADHD assessment, diagnosis, and treatment
  • Paediatric public health nursing
  • Well child nursing

She began her nursing training in the Philippines and completed her Bachelor of Nursing in New Zealand, followed by a Master of Nursing degree. She qualified as a Nurse Practitioner in 2023 and practise within full authorised NP scope.

Connected Minds was established to provide timely, thorough, and compassionate care for individuals and families who are navigating neurodevelopmental and mental health challenges. Her approach is neuroaffirming, whānau-centred, trauma-informed, and evidence-based, with a strong emphasis on collaboration and clarity.

She believes meaningful assessment goes beyond symptoms — it requires understanding the person, their family, their culture, and their environment. Her role is to help people make sense of their experiences and to support practical, sustainable pathways forward.

Jam is a member of NPNZ, the College of Nurses Aotearoa, the Association for Child and Adolescent Mental Health (ACAMH), the Australian and New Zealand Mental Health Association (ANZMH), the Australian ADHD Professionals Association (AADPA), the Attention Deficit Disorder Association (ADDA), and the International Marcé Society for Perinatal Mental Health.
