Service & Fees

At Connected Minds, we provide evidence-based ADHD and mental health assessment and care for children, adolescents, and adults. Our clinic operates within a structured, prescriber-led framework grounded in diagnostic integrity, safe prescribing, and continuity of care.

Assessments are delivered through a comprehensive clinical process led by a Nurse Practitioner with prescribing authority. I provide both diagnostic assessment and, where clinically indicated, medication initiation and ongoing management. This integrated approach allows diagnosis and treatment planning to occur within one service — reducing delays and fragmentation of care.

A diagnosis is made only where DSM-5-TR criteria are clearly met following thorough assessment and careful differential diagnosis.

Where appropriate, treatment may include psychological strategies, structured follow-up, or medication management. Assessment, diagnosis, and prescribing remain distinct clinical decisions to ensure safety, clinical integrity, and informed consent.

All fees are exclusive of GST.

Fees are payable on the day of service unless prior arrangements have been confirmed.

ADHD Diagnostic Pathway

Stage 1 – Pre Assessment Screening | $60
Non-refundable

This stage ensures assessment time is used effectively and allows early identification of complexity.

Includes:

  • Clinical Intake Questionnaire (including consent)
  • Standardised ADHD screening tools
  • Other required screening tools
  • Other collateral questionnaires (when required)
  • Scoring and interpretation
  • Clinical integration into formulation
  • Documentation within diagnostic report

Once payment and completed documentation are received, an appointment will be offered.

Stage 2 – Initial ADHD Assessment 60 minutes | $350

This appointment determines whether a full ADHD diagnostic assessment is clinically indicated.

Includes:

  • Comprehensive clinical interview
  • Review of childhood onset and developmental history
  • Educational and occupational history
  • Functional impact assessment
  • Screening for co-occurring conditions (e.g., anxiety, depression, trauma, substance use, sleep disturbance)
  • Risk and safety screening
  • Explanation of assessment pathways and next steps

This appointment does not provide a diagnosis. Further diagnostic assessment requires separate consent.

Stage 3 – Comprehensive ADHD Diagnostic Assessment

Child & Adolescent | $800

Adult | $580

A comprehensive ADHD diagnostic assessment consistent with DSM-5-TR criteria

Includes:

  • Structured diagnostic interview, feedback session and treatment planning (Child & Adolescent – 90 minutes | Adult – 60
    minutes)
  • Review and scoring of standardised ADHD questionnaires
  • View of available school reports
  • Differential diagnostic consideration
  • Diagnostic formulation
  • Written ADHD diagnostic report, suitable for GP and school use (for Child & Adolescent) or workplace documentation (Adult) if required
  • Clear recommendations for home and school supports
  • Where clinically appropriate and consented, medication initiation may be discussed and commenced (if decided on
    this on the day of appointment)

Not included:

  • Another appointment for medication initiation
  • Classroom or school observation (available if clinically indicated and discussed in advance)

Medication prescribing is not automatic and requires separate clinical consent.

Complex ADHD Diagnostic Assessment

Child & Adolescent | $1,000

Adult | $780

For children, adolescents and adults with higher clinical complexity (e.g. trauma history, neurodevelopmental complexity, multiple comorbidities, or significant school-based needs).

Includes everything in the Standard Package, plus:

  • Extended assessment and consultation time (additional 30-60 mins)
  • Detailed medication readiness and risk assessment
  • Comprehensive discussion of pharmacological and non-pharmacological options
  • Review of multiple collateral sources (e.g. OT, SLT, CAMHS, private providers)
  • Preparation for safe medication initiation
  • Enhanced diagnostic documentation
  • GP communication summary where clinically appropriate

School observation may be recommended in some cases and will be discussed and costed separately if required.

Additional Time (If Required)

In some cases, additional time may be clinically necessary to ensure safe and thorough assessment. Any additional time is discussed and agreed in advance.

Stage 3 – Comprehensive Autism Diagnostic
Assessment | $900

A comprehensive Autism diagnostic assessment consistent with DSM-5-TR criteria. ADOS-2 and ADI-R assessment will be completed, as part of this.

Includes:

  • Part 1 – 60 to 90-minutes structured diagnostic assessment (ADOS-2)
  • Part 2 – 90-minutes ADI-R parent discussion and feedback session and treatment planning
  • Review and scoring of standardised Autism questionnaires
  • View of available school reports
  • Differential diagnostic consideration
  • Diagnostic formulation
  • Written Autism diagnostic report, suitable for GP and school use (for Child & Adolescent) or workplace documentation (Adult) if required

  • Part 1 – 60 to 90-minutes structured diagnostic assessment (ADOS-2)
  • Part 2 – 60-minutes developmental history discussion (ADI-R)
  • Part 3 – 90-minutes feedback session and treatment planning

Comprehensive Autism & ADHD Diagnostic Assessment PACKAGE | $2,250

Full Payment is due on Initial Assessment Day

Includes:

  • Comprehensive Autism & ADHD Diagnostic Assessment as noted above
  • Pre-assessment Screening
  • 60-minutes Initial Assessment

Medication Management (ADHD & Mental Health)

Medication is offered only where clinically indicated and with informed consent. Prescribing decisions are independent clinical determinations based on diagnostic findings and risk assessment.

Phone consultation & follow-up 20 minutes | $120

Phone consultations are available for brief clinical matters that do not require a full appointment. These may include medication queries,
clarification of treatment plans, review of symptom updates, or coordination with other providers.

Phone consultations are structured, time-limited clinical interactions and are documented within the client record.

Mental Health Assessment

Stage 2 – Initial Mental Health Assessment 60 minutes | $350

A comprehensive first assessment to understand presenting concerns and determine appropriate next steps.

Includes:

  • Comprehensive clinical interview focused on presenting concerns
  • Overview of current symptoms and functional impact (home, school, work, relationships)
  • Brief psychosocial and relevant developmental history
  • Mental health screening (e.g., mood, anxiety, behavioural concerns, trauma history where indicated)
  • Risk and safety assessment
  • Review of previous supports or treatments
  • Initial clinical formulation
  • Discussion of recommended next steps
  • Treatment pathway explanation (including follow-up, therapy-informed support, or medication discussion where clinically appropriate)

Stage 3 – Comprehensive Mental Health Diagnostic Assessment

For more complex presentations or where diagnostic clarification is required.

Includes:

  • Extended clinical interview
  • Detailed psychosocial and developmental history
  • Review of presenting concerns and functional impact (home, school, work)
  • Risk and safety assessment (including self-harm/suicide risk where indicated)
  • Screening for co-occurring conditions (e.g., anxiety, mood disorders, trauma-related conditions, neurodevelopmental factors, substance use, sleep concerns)
  • Differential diagnosis consideration
  • Clinical formulation
  • Feedback discussion and treatment planning
  • Written clinical summary or diagnostic letter suitable for GP or insurer
  • Clear recommendations for next steps (psychological strategies, supports, referrals, or medication discussion where
    appropriate)

Additional Time (If Required)

In some cases, additional time may be clinically necessary to ensure safe and thorough assessment. Any additional time is discussed and agreed in advance.

Ongoing Non-Medication Support

Coaching / Therapy / Support Appointments

These appointments are available for clients and whānau who have completed an assessment through Connected Minds and require ongoing review, structured support, or coaching/therapy sessions

45 minutes | $200

NP Jam Jones specialises in:

  • ADHD/Executive Function Coaching
  • Parenting Support/Attachment
  • Anxiety Management
  • Mood Management
  • ACT (Acceptance Commitment Therapy)

These sessions provide structured clinical follow-up within NP scope and may complement medication management where indicated.

Short-Term Support Package (5 sessions) | $900

  • 5 × 45-minute sessions
  • Goal-focused intervention plan
  • Progress review
  • Written summary and recommendations (if required)

This package is particularly suitable for anxiety, adjustment difficulties, and mild-to-moderate depressive presentations.

Phone consultation & follow-up 20 minutes | $120

Phone consultations are available for brief clinical matters that do not require a full appointment. These may include medication queries, clarification of treatment plans, review of symptom updates, or coordination with other providers. 

Phone consultations are structured, time-limited clinical interactions and are documented within the client record.

Late Cancellation Fee

Applies to all appointments cancelled within 48 hours (2 business days – excludes weekends and public holidays)

If you cancel your appointment within 48 hours, a cancellation fee of $250 will be deducted from your appointment booking deposit.

Did not Attend Fee

Applies to clients who do not attend scheduled appointments and do not contact our clinic to advise that they will not be attending.

Important Information

  • ADHD, Autism and Mental Health assessment, diagnosis, and medication management are delivered as distinct clinical services.
  • A diagnosis is made only where clinical criteria are clearly met following comprehensive assessment.
  • Medication is prescribed only where clinically indicated and with informed consent.
  • Fees reflect specialist clinical expertise, diagnostic accountability, prescriber responsibility, and professional documentation standards.
  • Some services may be eligible for private health insurance reimbursement, depending on your policy. We recommend confirming coverage directly with your insurer prior to booking.

Payment Terms

  • Fees are payable on the day of service or the day before for online payment.
  • Certain fees (including pre-assessment screening and deposits) are required in advance.
  • Payment is accepted via online banking.

Pre-Assessment Screening

  • The pre-assessment screening fee is non-refundable.
  • Once payment and completed documentation are received, an appointment will be offered.

Appointment Deposit

  • A deposit of $300 is required three weeks prior to your appointment to secure your booking.
  • This deposit is refundable up to 48 hours (2 working days) prior to your appointment.
  • The remaining balance is due on the day of service.

Cancellations

  • Cancellations made within 48 hours (2 working days) will incur a $250 cancellation fee, deducted from the deposit.
  • Non-attendance will be invoiced at the full appointment fee.

Funding

Financial assistance for assessment and treatment may be available through:

  • Work and Income New Zealand (WINZ)
    Eligible families may be able to access support such as Disability Allowance, Special Needs Grants, or assistance with medical costs. Eligibility is income-tested and assessed on an individual basis. Families are encouraged to contact WINZ directly to discuss their circumstances. We can provide quotes and clinical letters to support applications.
  • Local Funding
    Support may be available for children and young people in the Rodney/Warkworth region. Connected Minds can also provide supporting documentation if required.
  • Insurance
    Some policies may cover ADHD or mental health assessments and follow-up care. We recommend checking directly with your insurance provider regarding your level of cover and pre-approval requirements. Currently, Connected Minds is not affiliated with Southern Cross Health Insurance.
